Please read these Terms of Service (“Terms”) carefully before using www.jillanamichelle.org (the “Site”) operated by Integrative Roads2Revival Health (“Company,” “we,” “our,” or “us”).

Scope of Practice & Disclaimer
Jillana Michelle is a licensed Family Nurse Practitioner in the states of Arizona and Oregon, where she has full practice authority. If you reside in one of these states, any one-on-one health consultations or services provided may be within the scope of a licensed healthcare provider.
In all other states or jurisdictions, Jillana Michelle acts in the role of a health consultant and wellness coach, not as a licensed medical provider. All services offered outside of Arizona and Oregon are educational, supportive, and lifestyle-oriented and are not intended to diagnose, treat, or replace medical care from your licensed healthcare provider.
Clients outside Arizona and Oregon must understand and agree that consultations are not medical advice, and participation does not create a provider-patient relationship under those state laws.
Privacy
Your use of this Site is subject to our Privacy Policy, which outlines how we collect, use, and protect your data. By using the Site, you consent to the practices described in that policy.
